Many thanks to those who have already submitted their vaccination status or requested a medical or religious exemption from the vaccination requirement using the COVID-19 Vaccine Certification Form, which you should have received via email on August 17. If you have already completed the form, please disregard this message.
For those who have not yet done so, we have extended the deadline to submit this information to Monday, August 30, 2021. While 72% of faculty and staff have already provided their information, we understand that a few employees may not have received the original email and a few others have asked for additional time to obtain information for medical exemptions, and so we have decided to give an additional week for people to respond. Please be aware that the vaccination requirement applies to all faculty and staff.
Please see the personalized email from “Lehigh Faculty and Staff COVID Vaccination Information via DocuSign” dated August 17 to complete this process. Automatic reminder messages from DocuSign will still indicate an August 23 deadline—please disregard that date.

You must be able to document at least partial vaccination status by the August 30 deadline, with a planned schedule for full vaccination.
By confirming your vaccination status, you are certifying the information you provided is truthful and accurate. The university may follow up to request additional verification at any time, including asking for a copy of your vaccination card.
